Who’s Eligible for an Adaptive Stroller?
Variety’s My Stroller® Program currently provides a Kid Kart Mighty Lite adaptive stroller for children who meet the following eligibility criteria:
Reside in one of the 71 counties that Variety currently serves, including: 59 counties in Pennsylvania (i.e. Adams, Allegheny, Armstrong, Beaver, Bedford, Blair, Bradford, Butler, Cambria, Cameron, Carbon, Centre, Clarion, Clearfield, Clinton, Columbia, Crawford, Cumberland, Dauphin, Elk, Erie, Fayette, Forest, Franklin, Fulton, Greene, Huntingdon, Indiana, Jefferson, Juniata, Lackawanna, Lawrence, Lehigh, Luzerne, Lycoming, McKean, Mercer, Mifflin, Monroe, Montour, Northampton, Northumberland, Perry, Pike, Potter, Schuylkill, Somerset, Snyder, Sullivan, Susquehanna, Tioga, Union, Venango, Warren, Washington, Wayne, Westmoreland, Wyoming, and York), and 12 counties in West Virginia (i.e. Barbour, Brooke, Doddridge, Hancock, Harrison, Marion, Marshall, Monongalia, Ohio, Preston, Taylor, and Wetzel).
Have a diagnosed physical, mental, or sensory disability documented by your child’s current licensed Physical / Occupational Therapist, Personal Care Physician, Physician Assistant, or Certified Nurse Practitioner.
Be 3 through 21 years of age. An application may be submitted up to 60 days prior to the child’s 3rd birthday.
Complete all applicable sections of the Program Application.
Applicant’s household income must meet the income guidelines for Variety’s programs (see pages 6-7 in the application for more details). Income verification will be required.

Letter of Medical Justification (LMJ)
Submit an LMJ from your child’s current licensed Physical / Occupational Therapist, Personal Care Physician, Physician Assistant, or Certified Nurse Practitioner indicating and detailing that a Kid Kart Mighty Lite adaptive stroller would be medically appropriate and therapeutic for the child. This same licensed professional should also complete and submit Variety’s My Stroller Order Form & Measurement Sheet (link to form is below).
A phone consultation with Variety’s Program Manager may be required with the parent and/or licensed medical professional writing the letter of medical justification for more information following the review of the application.
Variety requires the applicant and his/her parent or legal guardian to attend a scheduled regional presentation to receive the equipment in order to properly review all safety and equipment specifics before it is taken home.Program Application Program Handout My Stroller Program Order Form
