Who’s eligible for an adaptive stroller?

Variety’s My Stroller® Program specifically provides a Convaid EZ Rider adaptive stroller to children with disabilities. All equipment is provided for free, but you must meet the following eligibility:

Location

Must live in one of 59 counties in Pennsylvania, including:  Adams, Allegheny, Armstrong, Beaver, Bedford, Blair, Bradford, Butler, Cambria, Cameron, Carbon, Centre, Clarion, Clearfield, Clinton, Columbia, Crawford, Cumberland, Dauphin, Elk, Erie, Fayette, Forest, Franklin, Fulton, Greene, Huntingdon, Indiana, Jefferson, Juniata, Lackawanna, Lawrence, Lehigh, Luzerne, Lycoming, McKean, Mercer, Mifflin, Monroe, Montour, Northampton, Northumberland, Perry, Pike, Potter, Schuylkill, Somerset, Snyder, Sullivan, Susquehanna, Tioga, Union, Venango, Warren, Washington, Wayne, Westmoreland, Wyoming, and York.

Diagnosis

Have a diagnosed physical, mental, or sensory disability documented by your child’s current licensed Physical / Occupational Therapist, Personal Care Physician, Physician Assistant, or Certified Nurse Practitioner.

Age

Be 3 through 21 years of age. An application may be submitted up to 60 days prior to a child’s 3rd birthday, but equipment can not be presented until the child is 3 years old.

Application

Submit the Program Application to the best of your knowledge.

Income

Applicant’s household income must not exceed Variety’s income guidelines. Income verification is required with the application.

-For each additional person, add $28,400.

Letter of Medical Justification (LMJ)

Request an LMJ from your child’s current licensed Physical or Occupational Therapist, Personal Care Physician, Physician Assistant, or Certified Nurse Practitioner indicating and detailing that a Convaid EZ Rider adaptive stroller would be medically appropriate and therapeutic for the child.

This same licensed professional must also complete Variety’s My Stroller Order Form.

Presentation Requirement

Variety requires the applicant and his/her parent or legal guardian to attend a scheduled regional presentation to receive their customized medical equipment, to conduct a final fitting, and to review all safety and equipment specifics before it is taken home.

Can we apply for a second adaptive stroller?

YES! If your child received an adaptive stroller through Variety and outgrew it, you are welcome to apply for a second stroller three-years after you received the first one. Variety can only fund a maximum of two strollers per child.

To re-apply, your child must still meet all of the eligibility guidelines , and a full application must be completed again (including a new stroller order form). We do not require the old stroller back.
